I’m reaching out to you in advance of my retirement on December 31, 2021 with a few important reminders about my practice transitioning to Kelsey-Seybold Clinic.
My practice will fully transition to Kelsey-Seybold Clinic on January 1, 2022. I hope you’ve had the opportunity to learn more about Drs. Adam Mitchell and John Ruelas, who will take over the care of my patients.

As of January 1, 2022 calls to my main phone line of 713-455-1306 will be received at the North Channel clinic of Kelsey-Seybold. Three of my former staff members (Lydia, Dora, Pearla) will be working at that clinic and can help. Also, any faxes sent to my fax line of 713-455-9560 will be received at that same location. Important Reminder for Medicare Patients
Kelsey-Seybold sees Medicare patients through several Medicare Advantage plans, including KelseyCare Advantage, WellCare Texan Plus, Humana Medicare and Aetna Medicare Advantage.
If you wish to transition your care to Kelsey-Seybold, our records show you must take action to enroll in a Medicare Advantage plan that is accepted by Kelsey-Seybold.
Kelsey-Seybold does not accept new patients with traditional Medicare. However, my patients are eligible to receive a one-year transition exception to receive care from Kelsey-Seybold using traditional Medicare until December 31, 2022. After that date, you must enroll in an accepted Medicare Advantage plan in order to continue care with Kelsey-Seybold.
